Woah, this looks different from the first NES one. I'm guessing it's the GameBoy version of Adventure Island II on the NES?

You are correct. It's scaled down a bit, but it's based on the NES Adventure Island II cart. The NES version is better, but this is a good game if you're looking for short, but very challenging levels. Think of it as a concentrated version of the NES release.
